Contingent Bill (signed by beneficiary)
Application / covering letter for reimbursement
These forms are submitted to the Parent ECHS Polyclinic where your card is registered.

Hospital Discharge Summary
Doctor’s Prescription
Investigation / Lab Reports
Referral letter from ECHS Polyclinic (if treatment was referred)
These documents confirm the treatment details and diagnosis.
💰 Bills & Payment Documents
Original Hospital Bills
Medicine Bills (with GST number)
Implant / procedure bills (if surgery done)
Receipts for payment
Bills must usually be original and stamped by the hospital or chemist. �
